Connecting Seniors and Carers Program

CareWays Connecting Seniors and Carers Program is a new initiative designed to support carers and seniors in Wollongong, Kiama, Shellharbour and the Shoalhaven.

As part of the NSW Government’s ongoing efforts to support older people and carers to live active and healthy lives. CareWays is one of four organisations selected by the Department of Communities and Justice (DCJ) to operate as ‘community connectors’ linking older people and their carers to vital services and community groups.

Carers and Seniors will benefit from access to wraparound support including:

Information, education and advice
Assessment of Needs
Outreach and engagement
Short-term case planning and coordination
Referrals to My Aged Care, Carer Gateway and the NDIS

CareWays has a strong local service footprint and we are well placed to empower seniors and carers to remain connected in their own communities that they know and love.

The new program aligns with the priorities in the NSW Government's Ageing Well Strategy and NSW Carers Strategy to promote social inclusion, community participation, and boost health and wellbeing.

Service features include linking older people and their carers to:

  • Local services designed to support wellbeing,
  • Independence,
  • And community connection

Seniors and carers may be linked to additional support including:

  • Financial wellbeing services
  • Legal services
  • Family and relationship support
  • Community engagement opportunities

The Connecting Seniors and Carers Program Coordinator provides outreach to older people and carers who may be socially isolated or unlikely to access aged care and community services without additional support.

CareWays is committed to supporting isolated seniors and their carers to plan more effectively for their aged care journey and avoiding the stresses associated with an unforeseen health incident. Research indicates without adequate in-home care and support seniors can end up in hospital prematurely and for long periods resulting in bed blocks.
